24 Hour Service
Typically, a family member serves as the primary caregiver and, when appropriate, helps make decisions for the terminally ill individual. Members of the hospice staff make regular visits to assess the patient and provide additional care or other services. Hospice staff is on-call 24 hours a day, seven days a week.
Coverage Area
Autumn View Hospice provides care to patients with a qualifying terminal diagnosis as evident from a hospice order provided by their physician or recent hospitalization. Autumn View Hospice services the community within 10 counties (Barrow, Clark, Cobb, DeKalb, Hall, Gwinnett, Fulton, Newton, Rockdale and Walton), and provides great customer services following all guidelines according to the conditions of participation in the State of Georgia. This description of services has been approved by the governing body of Autumn View Hospice and meets standards for hospice practice. This description of services shall be made available to all patients and their families.
Hospice Care for Cancer Patients
Hospice care usually begins at the point where medical professionals and patients agree that the cancer can no longer be cured and the patient has a prognosis of six months or less. Rather than continue treatment, the focus shifts to quality of life, pain relief, and emotional and spiritual support.
Our aim is to make sure those in our care have the most comfortable, richest experience possible towards the end of life. This can involve help at home from medical professionals, social workers, chaplains, bereavement specialists, and sometimes volunteers.
Research cited by the National Cancer Institute has shown that hospice care can improve quality of life for terminal cancer patients.
Levels Of Care
The Medicare Hospice Benefit affords patients four levels of care to meet their clinical needs: Routine Home Care, General Inpatient Care, Continuous Home Care, and Inpatient Respite Care. Payment for each covers all aspects of the patient’s care related to the terminal illness, including all services delivered by the interdisciplinary team, medication, medical equipment and supplies.
